Please see a Doctor to manage the symptoms, some of which may need medication, such as to regulate your blood pressure. Your Doctor will do a physical exam to advise you on the length and severity of the symptoms. Sometimes people need to be in the hospital for severe withdrawal.

Your main problem is the cocaine, and this needs attention from a drug rehabilitation program. If you are, as described, a "very heavy user", this will not be easy but is worthwhile to prevent future cardiovascular and cerebrovascular consequences. So, if you are dedicated to getting back on track, which appears to be the case, get proactive and address your issues.
