Many things: There are many possibilities that need to be worked through before a final diagnosis can be given. Some possibilities are gastroparesis, impaired bowel motility, infection, ulcers, irritable bowel syndrome, and others. Testing such as routine labs, thyroid tests, imaging and endoscopy may help in narrowing these down.
