A heart attack is extremely unlikely in a 23-year-old. Usually a cholesterol level is unnecessary at that age. Lipase is an enzyme and it is not related to cholesterol. Not having a test for cholesterol is not a matter of concern. Lipase is not a "cholesterol thing".
