Tinnitus is not a listed side effect of hydroxyzine by PDR.NET or DRUGS.COM so I am not sure it may worsen tinnitus or not. In general, medication's side effect will resolve with discontinuance.

The intensity and volume of tinnitus can vary day to day, even hour to hour. Doubtful that your one dose actually caused any deterioration. Contact an otolaryngologist and consider the use of a tinnitus masker device.
