You could have hallux limitus. Normal bone normal foot structure is present. It has to to do with first ray function. When weight bearing the first metatarsal elevated causing jamming big toe joint. Custom orthotics supporting first cuneiform joint to allow first metatarsal plantarflextion allowing normal motion first metatarsal phalangeal joint
