Get professional care ASAP. Condoms are not perfect; you're still at high risk for STIs and HIV and these are typical STI symptoms in MSM with multiple partners. You need prompt professional evaluation, including HIV and syphilis blood test, urethral and rectal gonorrhea and chlamydia tests, and throat testing as well (assuming you also have oral sex). Also consider PEP or PrEP to prevent HIV.

You might have traumatic proctatitis (inflammation of the anus and rectum) and traumatic prostatic inflammation. Stop the sex. Try sitz baths and take a stool softner. If not better in a few days pl see your doc
