If someone over exfoliated their nose super harshly to where the skin is thinner, how does one know if they injured only the epidermis or the dermal layer too? there was no blood but the skin has not thickened back up after 1 year. more time?

Would need to know what products were used to exfoliate. Th fact that there was no bleeding would suggest that here was very little dermal damage. See dermatologist for evaluation and possible treatment
