My reading reveals that there's more than a few pathways to myelofibrosis that lead to production of cytokines and are independent of JAK dependent pathways. The biology of myelofibrosis is certainly more complex than has been previously believed. It's important that the treating hematologists follow the latest research. Treatment designed to interrupt JAK pathways may not be enough in some cases.
