Are you following up regularly with a physician trained in addiction medicine? Multiple components including talk therapy, pharmaceuticals, alternative medicine (i.e. acupuncture), and various elements of social support are required to control addiction. This is a chronic illness with occasional setbacks, but you can maintain a normal life with close follow up with your care team.

I think your motivation is very genuine, and you are searching for a support system. Not unusual to experience challenges and some approaches to consider, if not done so far: Find an occupational physician who is experienced in managing addiction, consider cognitive-behavioral therapy, hypnosis and/ or biofeedback therapies may work.
