It could be inflammation or infection of the semen pathway. The best way to investigate and treat your problem is to seek consultation with a Urologist. Your primary care doctor(PCP) can arrange the consultation if you like. Perhaps U visit the PCP first

During sexual stimulation hyperemia occurs in all sexual organs. Back of the testicle is the epididymis. If pain keeps recurring you need to be examined by a Urologist.
