How much difference would a diagnostic mammogram make compared to 3d screening mammogram for someone with focal pain, new lump, and extremely dense breast, and normal previous annual mammograms?

Digital mammography is fairly accurate, especially with "focal pain and new lump". If your family history has women with breast cancer, an MRI may be necessary for accurate diagnosis, with "extremely dense breast" tissue.
