One might be dealing with Neuropathic pain. There are many potential causes, though Diabetes is high on the list. This is a good chance to have the PCP do a comprehensive, preferably 'Hands-On' Evaluation and follow treatment and management as recommended.

Your pain sounds concerning for cervical and lumbar radiculopathy. Often times when you have a herniated disc in your neck or back it can causing shooting (radicular) pain into the arms or legs. Symptoms can include: numbness, tingling or weakness in the extremities. Advanced imaging with MRIs would be indicated to evaluate if you would be a candidate for injections vs. surgery.
