Thyroid disease would definitely be in the differential among other conditions. Start with seeing your primary care doc, establish with one if not established. Depending on exam and detailed history, tests will be ordered that will help substantiate a diagnosis.

Gaining 40 pounds will cause your BMI to go up, and will affect your overall health in a negative way. You need a complete exam and blood work as well as a controlled diet. Go now, the excess weight can affect tour heart.

It is not feasible to provide an opinion without additional history, physical examination may be some tests. It would be prudent to see your doctor to see if you may be depressed. Depression is treatable. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water.
