The decision about how to treat varicocele (which is a collection of blood vessels in the testicular sac) usually depends on 2 things: the size of the varicocele & the sperm count of the patient. If you have not had a sperm count/semen analysis yet, that would be helpful. If it is normal there is no urgency to do surgery but if not, since you are young, surgery should be seriously considered

About 15-20% of men have a varicocele. If you do not have pain and the testicle on the side and your sperm count is normal you do have to have surgery. Not everyone with it has infertility. But if the size of the testicle is affected and the sperm count is abnormal. then surgery is helpful. Long time varicocele may lead to low sperm count but this is not definite and cant be predicted.
