I'm not sure this has ever happened. Certainly no more than one or two cases world wide. If not vaccinated, your risk of dying of COVID is far higher (maybe hundreds of times higher) than the possibility of dying from the vaccine.

COVID-19 vaccine is available, safe, and found to be effective for children 6 months and older and adults. In the US, it is emergency use authorized for 6 months and up and fully authorized for 16 years of age and up (Pfizer); 18 years and up for Moderna. I encourage everyone who is eligible to be vaccinated and boosted! (July 2022)
