You must undergo an examination by a physician to determine the nature of this swelling. If it's fluid you would have advanced congestive heart failure, end stage liver or kidney disease or severe hypothyroidism which doesn't appear likely. Can it be fat deposition from substantial weight gain? The MD will order appropriate tests depending upon the assessment. You may be reffered to specialist
