Discharge as described is usually a bacterial infection. If no antibiotic sensitivity was done, resistance to a particular antibiotic will reduce or eliminate effectiveness of treatment. A repeat culture/sensitivity may be necessary.

Oral sex can result in nongonococcal urethritis (NGU), probably usually due to non-STI oral bacteria. Doxycycline is recommended when azithromycin isn't effective. But don't get over-worried: NGU from oral sex is generally believed to be harmless.
