My ankle/foot is red swollen painful and tender to the touch the three different spots that are swollen have a white head in the dead center of them what could this be?

Could be an insect bite with a local reaction or injury or infection starting with a scratch or pimple. Please see a doctor in person to have this evaluated.

Sounds like infected areas that need attention. Could be that there was a bug or spider on your foot that bit you multiple times, and now areas are becoming infected or inflamed. You should see a medical provider to determine if further treatment is needed.
