Doc, im in the army and have developed a severe hemorrhoid that is painful when running, it is external and bulging. how severe of an issue is this? what steps should i follow for treatment? prep h and tucks pads havent helped?

Unfortunately, a surgical treatment for a thrombosed or "bulging" hemorrhoid is at least as symptomatic (painful!) as having the hemorroid itself. Topical relief for ease of uninterrupted sleep is possible with lidocaine ointment, but prep H and tucks will usually not help much.
