From your description, he has congestive heart failure from either severe aortic insufficiency, or uncontrolled atrial fibrillation or a combination of both. With a bicuspid aortic valve, a dilated or aneurysmal aortic root is common. He should be examined and evaluated by a cardiologist and a surgeon. Bicuspid aortic valves can be treated surgically if patient a good candidate. Do this ASAP.
