Pain can occur from rupture of an ovarian cyst, but seldom "like clockwork" on alternate sides. Endometriosis can certainly cause pain at the outset of menstruation, but is usually bilateral, and at 30, a history may include loose bowels concurrently. See your gyn provider for an exam and ultrasound testing.
