Do you have ulcer in the area? It is best to have a doctor evaluate you about what you could do to speed the healing of and to clean out (debridement) including the worms and probably be on device or medication that would allow speedy healing and preventive measure against re-occurrence. These sound like pinworms. They can migrate into the urethra but generally are seen only in the anus and cause itching. They don't cause any serious problem as a rule, but if they migrate into the urethra, urinary symptoms could be problematic. You may have maggots in chronic skin ulcers however if you don't have pinworms. Pinworms which usually clear up with good hygiene after bowel movements.
