If symptoms (occasional spotting in weeks 3/4 of cycle) are present for 4+ years, would any malignancies or something unusual be picked up on most recent trans-vaginal ultrasound (with saline)?

"Occasional spotting" late in the menstrual cycle may occur from cervical inflammation, which would not be seen via a T-V U/S, but fibroids, and thickened endometrial tissue would be seen on a T-V U/S. If your pap smears are normal, and the T-V sonograms are unremarkable, malignancies are generally not the cause.
