The presence of a single testicle within the scrotum can be caused by failure to descend (Cryptorchidism) or loss (monorchidism). If a testicle has been lost, the functionality of the one left typically increases to produce near normal levels of testosterone, and sperm. Normal sexual activity and fertility are typical. In undescended testicle, careful monitoring will catch early malignancy.
