An area of focal irritability, which has not responded to medications, or a vagal nerve stimulator, might be an area for extirpation. Most patients do not have profound adverse events, and many do indeed have fewer or even NO seizures. Discuss issues in detail prior to surgery, including expectations, complications, etc.
