Wouldn't know: This is a public information site where volunteer docs answer questions about health issues based on symptoms you post. It does not offer notes/scripts/referrals or other forms of direct care. Your regular doc may have a system for providing such a note. As a legal document I doubt you can get one from someone who does not know your case.
