Testicular pain : In my opinion, vitamin B12 deficiency or insufficiency does not cause testicular pain. If a person suggests testicular pain is do to vitamin B12 deficiency, a second opinion is strongly suggested. Testicular cancer detected early has an extremely favorable prognosis. Sometime a full work up may not reveal the cause of testicular pain & cam may be tried; be sure to check your B12 level first.

Unlikely : My first thought would be an epidimitis brought on by some viral or bacterial agents. I would have him see a urologist in the next day or so and see for sure.I can't buy into the B12 thing.
