Wrong Assumption: Assume the first responder is virus free, vs infected. Health care workers are tested often to ensure they are healthy. So assuming they are virus free, your chance of infection is zero.

COVID: Assuming someone was an asymptomatic infected individual, wearing PPE and coming within 6 feet of the patient for less than 15 minutes (arbitrary time defining exposure), if the patient was also wearing a mask up until swabbed, the possiblity is very low. The virus is spread via respiratory droplets. Assuming someone had a fever and a cough and loss of taste, s/he would not be swabbing.
