Orthodontist?: Are you sure he/she didn’t refer you to a hematologist? Call your Dermatologist to make sure exactly the referral path.

Splinter hemorrhage: I would guess he looked in your mouth and saw bad teeth gums. Splinter hemorrhages can be a sign of infectious endocarditis which may come from dirty teeth bad gums. But you need to ask him.
