The usual way: Hospital records are separate from office records. Most people have no need for hospital records as they are extensive and unreadable by non-medical people. A hospital "discharge summary" is sent to the primary care doctor who adds it to the office record. One person (spouse of adult, parent of child, etc.) just signs a request for records at the office, and a set of records is prepared and given.
