33 yo adult male with currently untreated adhd due to lapses in insurance, previously on adderall for years. saw a psych pa who wants to start with strattera, however i'm concerned due to having a "coarse liver with fatter infiltrates."

Legitimate concern: Given your underlying condition, i.e., fatty liver, Adderall (dextroamphetamine) or methylphenidate would be preferable to atomoxetin which has a potential to cause severe liver damage.
