Urologist: The problem may be the tight foreskin( phimosis) as well as inadequate lubrication of your partner's vagina. This friction can irritate your foreskin and penis tip. I recommend consulting a urologist to determine whether a circumcision is advisable or whether there is any foreskin disease. In the meantime have your partner and you use a lubricant like KY jelly or white Vaseline.
