My total testosterone is 470 ng/dl and my free testosterone is 88.9 pg/ml. i was told my optimal should be in the 600's. could this cause fatigue?

Normal: If your lab was drawn in the morning and has been repeated a second time to make sure the results are accurate consistent -- then your levels are normal and nothing more needs to be done. You do not need any replacement and stay away from pills and supplements on the internet. Who knows what's in them and what they could do to harm you. They certainly won't help you.

Testosterone Levels : I would not blame your testosterone levels as the cause of your fatigue. These are very good numbers to have despite your age. (they will fluctuate widely depending on the time of day). The free testosterone level is the telling piece of this puzzle.

Male hypogonadism : Treating these numbers without further information is not suggested. Many things can cause fatigue including low testosterone levels. In a 28-year-old male, nocturnal tumescence (early morning erections) is the best indication for low testosterone level. Would need more than total and free testosterone to make an accurate determination.
