Myofascial dysfunct: Myofascial dysfunction in the scapular support muscles attaching to the cervical spine and skull can produce these symptoms. Topical massage, rubifacients (Ben gay type), ice packs, local vibration all are helpful. Support pillow between shoulder blades at night.
