My periods are preceded by a weeks worth of light cramping, and sometimes incontinence (i have problems with oab and periods make it way worse). when my period finally starts, it lasts 1-2 days and is accompanied by unbearable pain, fever and nausea?

Hormone issues: can cause heavier, and or longer periods, could be caused by insulin resistance/polycystic ovarian syndrome, or other issues affecting ovulation. Endometriosis, is also a strong possibility as well, an exam with your doctor is an important next step.
