Several things: First an inadequate amount of vitamin d in the diet, or the inability to absorb vitamin d and fats (d is fat soluable). Also, inadequate exposure to sunlight, which converts vitamin d to its active form. Being a radiologist and working in a dark room all day, i'm surprised more of my colleagues don't have it!
