Anytime, but: The scientific literature shows that it does not happen until about age 30, and is still rare until about age 45. Cases in children or people in their 20's are so rare that they may get written up in a scientific paper. So, that means a person with some odd neurological symptoms but is under age 30 likely can put ALS at the bottom part of her list of possible diagnoses. Rare things are rare.
