Middle ear effusion: A common cause for your "base drum" sensation would be fluid behind the tympanic membrane. This will cause internal sounds to be heard. Your "base drum" is likely your pulse. The fluids collects secondary to allergy or viral or bacterial cause. If fever or persistent you should get checked.
