Polycystic ovaries: PCOS is associated with high androgen levels derived from ovary or adrenal cortex. Serum LH is elevated but FSH is normal.
Ovarian production of androgens due to ovarian insulin resistance. High circulating androgens/insulin resistance impairs normal ovulatory cycle.
