Multiple issues but: A teen boy would have been getting annual check-ups. His pediatrician would have checked for proper pubertal growth of the penis and testicles at each annual exam... that's the usual care boys get. In any case, a full exam is needed now by an adolescent or family medicine doctor, and maybe an endocrinologist. If one doctor does not carefully check the penis and testes, see a second doctor.
