Hard to say: Each of the features mentioned are part of many overlapping syndromes. A visit to a clinical geneticist (found at medical schools or children's hospitals) would be a way to sort this out. More information: Your broad outline suggests one of many syndromes. Without imaging, physical examination, and possibly lab tests it's impossible to tell. I agree with Dr. Ferguson that an evaluation by a medical geneticist or by a facility, such as a children's hospital, are the best places to go to seek more information.
