Aortic dissection : This condition can cause severe chest pain that radiates through the chest into the upper back and in between the shoulder blades. Other much more benign causes can cause this as well. But a male in his 30's may be suffering from a hereditary condition which can present with aortic dissection as it initial manifestation. This sort of severe pain is a medical emergency which require an er visit.
