Body position: Blood clots in the legs or abdomen (deep venous thrombosis, DVT) can break loose and lodge in the lungs (pulmonary embolism). Clots can form when blood flow is restricted, like when sitting for prolonged periods during a flight. Blood flow is normal when lying flat, so no problem when sleeping in a bed.
