Not always: Usually no effect on QTc. Even when it does so, the actual risk for serious heart rhythm is extremely low, especially for short courses of treatment up to 2 weeks. If prescribed clarithromycin, take it as directed and without worry. If further questions about this, discuss with the doctor or clinic who prescribed the drug.
