Urethral discharge: Most urethral discharge in men is due to STD, typically gonorrhea, chlamydia, or nongonococcal urethritis (NGU), but do not cause intermittent on/off symptoms. Other non-STD causes are quite uncommon, but possible -- e.g. prostatitis. None of these are common causes of pain in the glans or anywhere else on the penis.
