Posture... asleep..: Most likely due to posture of Hand relative to Head, neck and shoulder while asleep. Sometimes it can be simple Strain/ Muscle spasm or even partial shoulder dislocation. Best to see PCP or Physical Therapist for simple reduction/ relaxation moves. Small pillow support between head and shoulder of affected arm helps sometimes.

Sympathies to you: This does sound like frozen shoulder. Similar to your previous symptoms? Not uncommonly this awful condition can affect you on both sides. DO's can address this as can physiatrists, prolotherapists, so see whoever helped you the first time around. Hope it goes away quickly
