Subclinical : All test have to be interpreted in Clinical context. What was the free T4?Are are you having symptoms of hypothyroidism? Even though the TSH was above normal of about 4.5,if no symptoms of hypothyroidism usually retest later and only start meds when above 10.Discuss with your physician to come up with plan.

It depends...: TSH is a surprisingly unreliable test; many who are hypothyroid have a normal TSH and some with high TSH are fine. If you have some of many symptoms of hypothyroidism (fatigue, dry skin, constipation, feeling cold, weight gain, muscle aches etc.) consider Rx- also if free T3 in lower half of normal range. Also check anti-thyroid antibodies.See https://tinyurl.com/nrww2qg & https://tinyurl.com/yyp9bkon
