Depends...: Each subspecialty in pediatrics manages patients with very rare diseases. A partial list of subspecialists includes hematology/oncology, endocrinology, nephrology, neurology, medical genetics, rheumatology, pulmonology, etc. Many of these doctors are found at pediatric academic medical centers.
