My son struggle with mental issues . he don't go out if he get nervous his hands shiver and sometimes it looks likes he just blocks out . is this signs of anxiety . he is very intelligent but come to write exams he fails ?

Have him evaluated: I would start with having his regular doc evaluate him for medical illness as a trigger to any of these issues. If s/he finds no medical issues they can refer him to a mental health professional for evaluation & treatment.
