Sounds like: dehydration could be a major contributing. He should try hydrating extensively before, during and after his travels to see if this helps. 20 ounces of water every two hours should do.

Back spasams: In addition to dehydration possibility, ask him to think about his lack of mobility in a car or airplane. After seeing his doc, he may find that a yoga or other stretching practice can be helpful. Peace and good health.
